CDL Labor Logistics is pleased to announced that we have a group health care program that will satisfy the Affordable Care Act.

The weekly cost for the program is very affordable and the premiums will be deducted from you paycheck pre-tax.  (this is taken from gross pay before taxes, not net )

• Plans renew with Capital Blue Cross effective 2/1/16
• Offering the same three plans PLUS an additional new plan!
• Open enrollment will run from January 6th – January 20th. All changes made will become effective February 1, 2016. If you do not elect a plan for open enrollment your current election will remain in place.
• Capital Blue Cross website to download claim forms and search provider network/drug formulary:  www.capbluecross.com

What do you need to do during the OPEN ENROLLMENT period?

– Complete a 2016/2017 election form and return to your employer by January 20, 2016

– If you are enrolling in coverage for the first time, you will need to complete a Capital Blue Cross enrollment form and return it no later than January 20, 2016.
